If you live within our practice area you are welcome to register with us.
In order to register with us you need to complete a registration form. You can download the form below and bring it in to the surgery or complete our online registration form. Please complete all sections of the application form, otherwise your registration may be delayed.
If you choose to register online please attend the surgery within 7 days with your identification – see information below for details of what is required and why we ask for this. Once you have provided your documents please allow 2 working days before contacting the surgery for a telephone consultation unless it is an emergency.
Identification requests
On your first visit to our surgery we will request proof of your identity, one with proof of address and one with a photograph.
Why?
This helps to ensure the correct matching of your details to any existing NHS record, enabling previous medical records to transfer smoothly between practices. We appreciate your assistance in this matter. Therefore when you attend to register we request, that you bring two forms of identification with you – one with your photograph on and one with your address.
If you are newly resident in the UK you may also be asked to provide evidence that you are working, studying or volunteering here.

Acceptance on to the list is as at the discretion of the Doctor.

What happens next?
- All new patients will be offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you are taking.
- Medical treatment is available from the date of registration
- Your medical records may take several weeks to arrive at the Practice
- There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area

If you wish to change your GP practice you can telephone NHS Lothian registration department on 0131 537 8488 for advice.
If you have been removed from a GP’s list, and cannot get another doctor to accept you, please telephone 0131 537 8473.
People who are homeless may access local GP services. Alternatively GP, nursing and mental health services are available at TAP (The Access Point), 17 Leith Street, Edinburgh and at the Cowgate Clinic, 20 Cowgate, Edinburgh. Advice regarding housing and social work services are also available at TAP (The Access Point).
