New Patient Registration

To register as a new patient at Muirhouse Medical Group, please come to Reception between the following time:

12.30pm - 5pm at the Muirhouse Main Surgery

9.30am - 12 noon at the Pennywell Branch Surgery 

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

In order to undertake the registration process you need to complete a registration form.  You can download the form below or collect one from reception.  Please complete all sections of the application form, otherwise your registration may be delayed.

In order to establish entitlement to NHS services we need to see identification before you can be registered.  Please bring two items of identification with you when you come to register, one of which should include your current address. 

If you are newly resident in the UK you may also be asked to provide evidence that you are working, studying or volunteering here.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you are taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
